How to Submit a Prenatal Sample

  1. Refer to Sample Requirements page
  2. Print and complete patient information and referring physician information on Signature Prenatal Requisition form.

  3. Indicate Payment method.
  4. Indicate analysis requested:
    Signature PrenatalChip® or SignatureChipWG™
    Analysis of over 70 recognized genetic conditions plus subtelomeric and pericentromeric regions. If the pregnancy has abnormal ultrasound findings or abnormal fetal karyotype, consideration of the SignatureChipWG may be warranted. Please call to discuss your options with one of our genetic counselors.
    Karyotype
    Can be performed sequential to or concurrent with Signature PrenatalChip® (Note: for all prenatal cases, karyotyping is recommended prior to or concurrent with microarray testing, or following normal microarray test results).
    Parental Study
    Provide name and SGL# of index case (typically the mother's name).
  5. Provide Indication For Study, including ICD-9 codes. Note: this information is essential. Failure to provide this information may delay processing and may impact correct interpretation of results.
  6. Indicate Sample Type.
  7. Ship sample with completed forms overnight at ambient temperatures to:

  8. Email Signature Genomic Laboratories the Courier Tracking Number of the parcel.


Results will be faxed and mailed according to the details given on the Signature Prenatal Requisition Form. In the event of abnormal results, the referring clinician will also be contacted by telephone. For questions, please call (509) 474-6840.
