How to Submit an Oncology Sample

- Print the Oncology Requisition Form.

- Complete these required items on the requisition form.
- Patient information.
- TRANSPLANT DONOR STATUS. Necessary for accurate analysis.
- Clinical Information. Please include supplemental materials such as cytogenetic test results and/or pathology information relevant to the study, if available.
- Service Requested. All testing offered at this time is full-service.
- Test requested.
- Signature Genomics offers ten options for microarray analysis, which can be ordered individually or several tests may be ordered at once.
